Our clinic is on the 6th floor of the Capital Plains Bank Building at 500 N. Shoreline Blvd. Suite 606
Once you enter, grab a parking ticket from this machine. Keep your ticket with you and bring to your appointment and we will validate your parking.
The garage entrance is off of State Spur 407 between N. Water St. and N. Chaparral St.
We offer complimentary parking in the dedicated parking garage across the breezeway.
Make your way across the breezeway on 2nd floor to elevator bank
Take the elevator to the floor six and make your way to the very end of the hall to find suite 606.
Come on in and make yourself at home. We’ll be right with you. Your next step will be to complete your Veteran History Form.
